SIH Memorial Hospital of Carbondale and SIH Herrin Hospital have received multiple national recognitions for helping patients with heart failure live longer and healthier lives.

According to the American Heart Association, more than 6.5 million adults in the United States are living with heart failure. Providing optimal health care to those patients requires staying on top of constantly advancing recommendations.

SIH Memorial Hospital of Carbondale and SIH Herrin Hospital each received the American Heart Association’s 2021 Get With The Guidelines® Heart Failure Gold Plus Quality Achievement Award. Research has shown the guidelines can lead to fewer hospital readmissions and lower mortality rates.

“Adhering to these guidelines has led to numerous improvements in the quality of life for our clinic patients. Our clinic provides an alternative pathway to managing CHF that keeps patients out of the hospital and leading productive lives,” said Amelith Brooks, Program Manager for SIH Cardio Clinic Services.

The award, which both hospitals also received in 2020, is an affirmation of the SIH Commitment to Care through the focused effort of ensuring heart failure patients receive the most appropriate treatment according to nationally recognized, research-based guidelines founded in the latest scientific evidence. The goal is to speed up recovery and reduce the chance those patients end up back in the hospital.

SIH Memorial Hospital of Carbondale and SIH Herrin Hospital earned the award by meeting specific quality achievement measures for the diagnosis and treatment of heart failure patients at a set level for a designated period. These measures include:

  • Evaluating and ensuring proper medication use and aggressive risk-reduction therapies.
  • Ensuring patients get education on managing their heart failure and overall health before they are discharged
  • Getting a follow-up visit scheduled

The honors don’t stop there. SIH Memorial Hospital of Carbondale and SIH Herrin Hospital were also recognized on the AHA’s Target: Heart FailureSM Honor Roll.

Hospitals are required to meet specific criteria that improves medication adherence, provides early follow-up care and coordination and enhances patient education. The goal is to reduce hospital readmissions and help patients improve their quality of life in managing this chronic condition.

SIH Memorial Hospital of Carbondale and SIH Herrin Hospital additionally received the Association’s Target: Type 2 Diabetes Honor Roll award for its work with diabetic cardiovascular patients.

Our comprehensive inpatient and outpatient programs are driven by best practice as we strive to provide education and treatment options for this chronic disease. This recognition is a public banner of the inner workings of this dedicated SIH team. The collaboration, resourcefulness, and compassion the CHF team possesses is evident in our awards and excellent patient outcomes,” Jessica Milburn, NP said.
